Street , Suite 1234, in the County of New York, City of New York, 11211, I served the within Subpoena Duces Tecum on Company XYZ,
known to me to be the Witness named herein by:
1- delivering and leaving with Janie Doe, corporation counsel, true copies thereof. Said Janie Doe is an expressly authorized agent for said
Company XYZ.

2-and by paying her the sum of $20.00 as fee for the production of records and for traveling to and from the place where she was required
by said subpoena to attend, and for one day's attendance.

General Instructions
Anyone NOT A PARTY to the action, who is over the age of 18, may serve the subpoena.
1-Find the person to be served.
2-Give the person a copy of the subpoena and the witness fees.
3-Fill out an affidavit of service for each person served.
4-The person on whose behalf the subpoena was served should retain this Affidavit of Service for further procedures if the person
subpoenaed fails to comply with the subpoena. .
